All-on-6 vs All-on-4 for the Upper Jaw: Which Is Better?

The short answer

The upper jaw is the one place where All-on-6 is more often preferred. Upper-jaw bone is softer and less dense than the lower jaw, the sinuses limit where implants can go, and the arch carries significant chewing force — so distributing the load across six implants can give a stronger, more reliable result up top. That said, All-on-4 still works very well in many upper jaws thanks to its angled implant design, and the choice always depends on your specific bone, shown on a CT scan. The lower jaw, being denser, frequently does excellently on All-on-4. Taki Dent assesses each arch individually.

Why is the upper jaw different?

The two jaws are not built the same. The lower jaw (mandible) is dense, solid bone that grips implants firmly. The upper jaw (maxilla) is softer and more porous, has the maxillary sinuses sitting above the back teeth limiting implant placement, and tends to lose bone faster after teeth are gone. All of this makes the upper jaw a more demanding place to anchor a full arch.

Because of that, the conversation about four versus six implants often plays out differently up top than it does down below.

Why is All-on-6 often preferred up top?

In softer upper-jaw bone, spreading the load across six implants rather than four means each implant carries less stress and the arch has more anchorage points in bone that holds less tightly. Six implants also add redundancy in the jaw where bone quality is less forgiving, and reduce reliance on the rear cantilever. For these reasons many specialists lean toward All-on-6 for the upper arch, particularly when bone density is borderline or the bite is heavy.

It is a tendency, not a rule — the deciding evidence is always your own CT scan.

Does that mean All-on-4 is wrong for the upper jaw?

Not at all. All-on-4 was designed precisely to succeed in challenging bone, and its angled implants engage the better bone at the front of the upper jaw while avoiding the sinus — so it works very well for many upper arches without grafting. Plenty of patients have a successful upper All-on-4. The point is simply that the upper jaw raises the bar, so the four-versus-six decision deserves extra care there. The lower jaw, by contrast, is dense enough that All-on-4 is frequently the natural choice. A specialist assesses each arch on its own merits from the scan.

Frequently asked questions

Does the upper jaw need All-on-6?

Not always, but it is more often preferred up top because upper-jaw bone is softer and the sinuses limit placement, so six implants distribute load more reliably. A CT scan decides.

Can I have All-on-4 on the upper jaw?

Yes — All-on-4 was designed for challenging bone and its angled implants work well in many upper jaws without grafting. The upper arch just deserves extra care in choosing.

Is the lower jaw better for All-on-4?

Often, yes. The lower jaw is dense, solid bone that grips implants firmly, so All-on-4 frequently performs excellently there.
